Making a claim is easy! Here are three things to check before you make a claim.

1. Check your cover
The  fund rules and your product guide outlines exactly what you are and what you’re not covered for. The product guide has handy information about annual limits and item limits on extras cover, inpatient/outpatient hospital services, and the clinical categories included in your cover.

2. Check your remaining limits
We know you won’t always remember what you’ve already claimed in the financial year, so check what limits you have remaining on your extras cover.

3. Get a benefit quote
 If you have a quote for your extras service, you can check how much we’ll cover and what your out-of-pocket expenses will be.

How to make a claim 

Extras claims

No matter how you claim, always keep your receipts for two years.

 The easiest ways to make an extras claim.

  1. On the spot claiming
     Just swipe your member card on the terminal available at most extras providers. It will apply your Defence Health benefit for the service and all you do is pay any remaining gap.
  2. Member Portal
    You can submit your claim via the Member Portal, for the fastest refund.

Or complete a claim form and submit it through one of these channels:

  • Email it with your accounts and/or receipts to claims@defencehealth.com.au
  • Fax it with your accounts and/or receipts to 1800 241 581.
  • Post it and your original accounts and/or receipts to Defence Health, PO Box 7518, Melbourne VIC 3004.
  • Attach your original accounts and/or receipts and take them to an Australian Military Bank branch. They will fax your completed claim form and receipts directly to us. A similar service is also provided by Defence Bank, but only if you are a Defence Bank customer.
